Pathological Review on Side-effects of Inhalation Anesthesia



Abstract
The :.authors studied the ultrastructural changes of rat liver and kidney following inhalation ane~thcThe results were summarized as follows:
Light microscopic examination of the live: and kindney reveals no significant chages in both group.
Eecron microscopically, in the liver cells immediately after halothanc. anesthesia, mild mitochcndri_il degenerations such as swelling, disappearances of cristae and granules and occasional. rupture of the outer membranes, slight decrease of rER with vesiculation, fragmentation and detsche.i ribosomes and mild proliferation of sER. Noted are focal cytoplasmic degradation and decrease of glycogen particles. In the 24 hours after anesthesiz, these changes were apparently minified, with trend to be normalize. In the kidney, immediately after anethesia, mild proliferation. of sER and slight degeneration of mitochondria were observed in the tubular epithelial cells of ?:he cortex and medulla. In the 24 hours after anesthesia, slight degeneration and diminution of ;nitochondria, moderate proliferation of sER, appearance of lysosomes in varying shape and focal thickening of basement Pnembrane were observed .in the proximal tubules. These changes were more prominent in the tubules of the medulla tl!an in the cortex. Immediately after methoxrflurane anesthesia, the liver cells showed swellings, chondriosphere formation and disappearance of cristae and granules in mitochondria, vesiculated rER with detachment of ribosomes, proliferations of sER, atrophy of Golgi complex, decreased glycogen particles and mild increases of lysosomes. In the 24 hours after anesthesia, these changes were also greatly minified_ In kidney, immediately after anesthesia, a mild proliferation of sER was observed in the proximal tubules, and mild degeneration of mitochondria and slight i~7crease of lysosomes were observed in the tulles of the medulla. In the 24 hours after methoxyflurane anesthesia, widening of the tubular lumen, swelling and degeneration of mitochondria, increase of varying shaped lysosmes, appearance of focal cytoplasmic degradation and mild thickening of basement membrane were observed in the proximal tubules. In the tubules of the medulla, marked proliferation o; sER, .increase of lysosomes, diminution of mitochondria, focal thickening of basement membrane and .marked diminution of basal intussusctption were observed.
The ;results obtained in this study suggest that in both halothane and methoxyflurane anesthesia, the liver cells revealed nonspeciffc, mild degenerative changes, principally in mitochondria and rER> which are almostly restored by 24 hours afl;er anesthesia. And metho-:y~lurane anesthesia causes focal thickening of basement membra~ze, diminution of basal intuss~.aception, and mild degenerative changes in the tubules of the cortex and the medulla. These changes are considered to be related with renal insufficiency induced by methoxyflurane anesthesia in human practice.
